How to Submit a Dispute

1. Complete the Submission Form

Provide basic information about the parties and a brief description of the dispute.

2. Include Any Relevant Agreements

If there is a written agreement referring disputes to Fidelis, please attach a copy. If not, the parties may still jointly agree to arbitrate under our Rules.

3. Pay the Filing Fee

A non-refundable filing fee of $2,000 is required to initiate arbitration. Payment instructions will be provided upon submission.

4. Confirmation and Next Steps

Once we receive your submission and payment, you will receive a confirmation email. We will then initiate the appointment of an arbitrator and notify the parties of next steps.
